In 2022, the Support Coordination Team underwent a review to ensure it was better able to meet the needs of clients, evaluating resources, policy, procedure, and local process. The review has increased productivity and improved services being provided to clients.

SEQUAL has a small and well-connected Support Coordination team and workloads tailored to a Support Coordinator’s skillsets; we are an agile team best placed to respond to the changing needs of clients and the unpredictable environment we have all experienced over the past two years.

The Support Coordination Team continues to demonstrate SEQUAL’s values; Support, Include, Empower in our day-to-day work. Most importantly, the team works with participants, families, and stakeholders to help create positive outcomes and increase participants’ quality of life. These have included:

  • Assisting with securing funding and sourcing much-needed Assistive Technology.
  • Providing guidance and clarity to people and families around how to best use their funding and what funding can be used for.
  • Increasing people’s capacity so they can have a heightened understanding of their plan funding, the portal and other provider platforms.
  • Assisting people and their families in gathering evidence to increase their funding and supports to reduce multiple risks, health decline and carer fatigue.

The Support Coordination team routinely supports participants by creating and monitoring plan budgets, connecting to services, and building an understanding of their NDIS Plan and funding.

Our skilled Support Coordinators assist people to navigate and implement their NDIS Plan.

How do I nominate SEQUAL as my Support Coordinator?

Identify SEQUAL as your chosen Support Coordinator in your planning meeting. Your planner will then send a request for service to us which we can accept, and a plan handover meeting will be arranged between your planner and us. We contact you as soon as possible after the meeting with the planner and meet with you to discuss what we can do for you and what you would like from your plan.
